• You can only receive documents by visiting the hospital (proxy receipt available) or by mail (registration).

• If you receive it by proxy, you need to know the exact name and date of birth of the person who was inspected.

★ Time required to issue the result sheet: It is flexible depending on the situation, so you can receive accurate information when you register.

What to Bring

  • Passport or passport copy
  • If you have received any documents or required inspection items, please make sure to print them out.
  • If you have a baby notebook or past vaccination record in addition to the vaccination record registered with the Korea Disease Control and Prevention Agency, please bring it with you. If you have served in the military, please bring documentation of the vaccination history you received while serving in the military.
  • If you had an antibody (Ab, IgG) test during a past examination, please bring the results.
  • If a person aged 0-5 is examined, please bring the result sheet of the most recent infant health examination with you.
  • If both left and right eye vision is below 0.5, make sure to bring glasses or lenses. If you wear lenses, make sure to bring a lens case.

Precaution

  • The required vaccinations vary depending on the requirements of the state and school in the United States. If you do not have the documents or conditions you received, you must find out the necessary information yourself.
  • Additional costs will be incurred if you request a change in documents after receipt. If you have any documents to submit, please make sure to print them out.
  • If you are replacing your passport with a new one, please be sure to tell us when applying.
  • If you have multiple nationalities, please submit the passport you want to fill in the document.
  • If you do not have an inoculation record, you will be tested for antibodies or vaccinated at our hospital.
  • Please avoid excessive exercise, overeating, drinking alcohol, or drinking protein shakes the day before your examination.
  • If the necessary tests include glucose and cholesterol tests, fasting for 8 hours is required even if you are not diabetic.
  • For women, please visit the doctor for a urine test 3 days after the end of your period. Other tests can be performed regardless of your menstrual period.
  • If you are pregnant or may be pregnant, please be sure to tell us when you apply.
  • If you are a hepatitis B carrier, please be sure to mention this when registering and seeing a doctor.
