    (Copies cannot be issued if not present)
  • In the case of receiving only CDs, the required documents shall be prepared for protecting personal information.
  • Since the patient's medical records are very sensitive, personal information cannot be provided via wire.

  • Those under 14 years of age
    The consent forms and letter of attorney shall be prepared by the legal representative. Submit documents to verify if the delegate is a legal representative such as family relations certificate.
  • Those aged 14-17 years of age with no certificate of residence
    Submit ID (student card, passport) or family relation certificate.
  • Those over 17 years of age
    Submit certificate of residence (Article 24-1 of Residents Registration Act)

※ Precautions for preparing required documents
  • An ID or its copy must contain a resident registration number and photo.
  • Documents issued by government offices, such as family relation certificates and copies of resident registration, are only valid for the last three months.
  • The paternal relation confirmation document shall specify the relationship between the inspector and the applicant.
  • The seal or thumbprint shall not be accepted, and the consent form and the proxy statement must be autographed by the patient.
  • The consent form should include the details (date, record range) for issuing the copy.
