- New Company : Business license, list of people subject to checkups (including resident registration number, date of employment, department), work environment measurement table, cost coverage notice (applicable to the company supporting cost)
- Existing Company : List of people subject to checkups (including resident registration number, date of employment, department), work environment measurement table, cost coverage notice (applicable to the company supporting cost)
- List of people who will have the health examinations at the workplace when including NHI program
- Reservations must be made in advance.
